The purpose is to add a person to the “Personnel” database and assign to the “Organization”
The upper portion of the form is for the personal information of the individual. The date field for DOB (date of birth) is limited to 14 years before the current date, as that is the minimum age for junior firefighters in PA. The Date Checked is limited to Today, allowing us to identify data that has not been updated within specific time frames.
The lower portion assignes the person to the organization, capturing organization specific assignments such as title, radio call, and current status.
